Two sessions, designed to fit a working week. The first session is the analysis. The second session, held 30 days later, is the review. Together they form a complete learning cycle.
Each session has a clear purpose and a defined structure. Here is what happens in each one.
Before the first session, we work with the team coordinator to identify two or three recent operational errors that are representative and relevant. These become the material for the workshop. The errors must be real and recent — typically from the previous two to four weeks. This pre-session step ensures the workshop time is focused on analysis, not on selecting what to discuss.
The full operational team gathers — operators, supervisors, coordinators, and relevant support roles. The facilitator guides the group through the four-question method for each selected error: What happened? Why did it happen? What did we do? What are we changing? The session produces a completed one-page document for each error analyzed, with a named responsible person and a specific deadline for each change.
The 30-day period between sessions is when the agreed changes are implemented. The one-page document serves as the reference. The responsible person for each action knows what they committed to and when it is due. No external monitoring is required — the document and the upcoming review session create sufficient accountability.
The team reconvenes to review the one-page documents from Session One. Did the agreed changes happen? If yes, has the error recurred? If not, what prevented implementation? This session closes the learning loop and often surfaces new insights about the original causes. It also reinforces the habit of following through on agreed actions.

The facilitator travels to your location. Recommended for teams where physical presence helps with group dynamics and where the operational context is best understood on-site. Available across Argentina.
Both sessions conducted via video conferencing. Works well for teams that are already comfortable with remote meetings and where travel logistics are a constraint. The one-page document is shared digitally.
Session One in-person (where group dynamics are most important) and Session Two remotely. A practical option when travel costs are a consideration for the follow-up session.
This workshop is designed for the complete operational team, not just management or a quality committee.
The people who were closest to the error — the ones who saw it happen, who had to deal with the immediate consequences — hold information that is not available to management. When the analysis is done only by supervisors or managers, it inevitably misses important details about what actually happened at the operational level.
Including the full team also changes the ownership of the solution. When operators participate in identifying the cause and agreeing on the change, they are invested in making that change happen. When the solution comes from management, it is management's problem to implement.
The method works with groups of 4 to 20 people. Larger teams may require adapted facilitation techniques to ensure everyone has a voice.
Session One: 2 to 3 hours. Session Two: 1 to 2 hours. Total time investment for the team is under half a working day across 30 days.
Sessions can be scheduled at a time that minimizes operational disruption — early morning, end of shift, or during a scheduled break in production.
